When coding, if the neoplasm type (primary, secondary) is not specified and is not obvious, should I just assume that it’s primary? 

And when primary and secondary are specified, do I code for both?

Also, is the sex of the breast is not specified when coding should I just assume it’s a female breast?

Don't assume anything! :)

If you have any questions, ask (email) your instructor.  They will provide the additional information that you need to code the case.

We cannot assume anything in the world of coding!  :)

Having said that, can you please send me an email with your questions along with specific examples of diagnoses?  Depending on the exact wording of the diagnosis makes it difficult to give a definitive answer to your question on when to select the primary or secondary behavior columns, so it would be better to go over what you are asking via email so that other students don't get confused in the process, especially if your questions are in relation to diagnoses that are on the coding exam.  Your other question does pertain to a diagnosis on the coding exam, so that also will need to be addressed via email.  Thanks!
